英国留学签证卡(VISA卡,非护照)丢失，如何补办

学生签证，今年4月发的，明年1月到期。因为9月中旬就要回去，所以比较着急。请问补办需要什么材料以及应该去哪里等相关信息。谢谢。

If your biometric residence permit is lost or stolen, you must report the loss or theft to us as soon as possible by phoning 0300 123 2412 from the UK or ([phone] from abroad. If you are abroad when your permit is lost or stolen, you should phone us before you try to return to the UK.

You must then use application form ICFN (RC) to apply for another permit. If you fail to do this within three months of reporting the theft or loss, you may need to pay a financial penalty of up to £1,000, or we may shorten your permission to stay.
